During the call, a Member Services representative will request that applicant provide a full Social Security Number (SSN), date of birth (DOB), and driver's license (DL) number (or another government-issued ID number, if applicable), as well as contact information (Full Authentication includes: Complete SSN, DOB, and Driver License Number).
